Hero's Forge ornaments were introduced via Patch in August of 2012 (during the Veil of Alaris expansion).

Armor ornamentations, aka Hero's Forge (armor) Ornamentations (Slot 21), should not be confused with Weapon, Shield & Bow Ornamentations (primarily Slot 20, and they will not go onto your Hero's Forge Key Ring).

Images below indicate that the ornamentation set / ornament is in-game verified. All Ornament sets have a related achievement, unless otherwise stated.

Ak'Anon Clockwork

These are sold by a mysterious presence (Plane of Knowledge), for Overseer Tetradrachms.

These ornament sets are related to (4) different Hero's Forge - Ak'Anon Clockwork Achievements (which are class specific, so you can achieve only one of them).

Ak'Anon is a zone in EverQuest (home of the Gnomes).

Overseer Tetradrachms are earned via the Overseer game (/overseer).

Ancient Ssraeshzian

These were once quested in The Overthere (original zone), via Norx Blackheart. However, as of about July 2013, he has only given the regular Ssraeshzian ornaments.

These sets do NOT seem to have any related Hero's Forge Achievements. However, the four sets do have the unique distinction of being wearable by any class (All / All).

Ssraeshzian is a reference to Emperor Ssraeshza, ruler of the Shissar.

Otherwise, there appears to be no way to acquire them anymore, except via other players who may still have them.

Arcron

These ornament sets are related to (4) different Hero's Forge - Arcron Achievements (which are class specific, so you can achieve only one of them).

These are, perhaps, named for High Arcron Zeklor, who is found in Gorowyn.

These can be crafted into the Fereth ornaments (see below).

Arx Mentis

They can be purchased from Researcher Orbiana  using Arx Mentis Crystals (raid currency).

Arx Mentis is a zone in EverQuest.

Auburn Hero's Forge

These are reportedly random global drops from Level 10+ MOB's, however, they do seem to drop most often in Veil of Alaris & Rain of Fear zones.

These ornament sets are related to (4) different Hero's Forge - Auburn Hero's Forge Achievements (which are class specific, so you can achieve only one of them).

Auburn is a color (reddish-brown).

These can be converted into the Old Auburn Hero's Forge ornaments by right clicking them (see below).

Beryl Hero's Forge

It appears these were once global drops, but now only drop in active Hot Zones. As an added challenge, it appears these drops may also have a "trivial loot code" connected to them, meaning the MOB's have to con Dark Blue or higher to your character.

These ornament sets are related to (4) different Hero's Forge - Beryl Hero's Forge Achievements  (which are class specific, so you can achieve only one of them).

Beryl is a mineral / gemstone that is transparent pale green, blue or yellow.

These can be converted into the Old Beryl Hero's Forge ornaments by right clicking them (see below).

Bixie

'Bloodforged' is a term that likely means crafted with blood

Dark Elf

These are likely named for the Di'Zok family. Historian Silren Di'Zok, Prince Selrach Di'Zok & Queen Velazul Di'Zok are in Chardok; while Ine'Thar Di'Zok is in the Plane of Justice.

Draconic Lava

These can be purchased from the Raid Vendor Morton Stonebreaker using Warforged Emblems (raid coin) in Western Wastes (CoV).

Draconic means harsh or severe. It also means dragon-like.

Duende

They can also be purchased from Riallion Cassand in Sarith, City of Tides using Rebellion Chits (raid currency); Olgor the Finder in The Resplendent Temple using Diamond Coins (raid currency); and Sholano in Erillion, City of Bronze using Bronze Fiats (raid currency).

Additionally, they can be purchased in the Day Break Marketplace (as a set).

These ornament sets are related to (4) different Hero's Forge - Eternal Grove Achievements  (which are class specific, so you can achieve only one of them).

An 'Eternal Grove' would be a stand of trees, that live forever. 

These can be converted into the Old Eternal Grove ornaments by right clicking them (see below).

Fereth

These are crafted from Arcron ornamentations.

These ornament sets are related to (4) different Hero's Forge - Fereth Achievements  (which are class specific, so you can achieve only one of them).

Fereth are Sarnak's in the Ring of Scale expansion.

They are made with one using an Arcron ornament and (1) to (9) Combine Viridian Chain, Cloth, Leather or Plate -- which, are also crafted using Sebilisian Viridian Pigment (dropped) and a Combine Chain Links Cloth Yard, Leather Square or Plate Sheet (sold by a records clerk in Lceanium) (Tailoring and / or Smithing Skill 450).

The book "Simple Work of the Sarnak Hero" (a random drop from named / rare MOB's in Ring of Scale) can be scribed to learn all of these recipes.

Festive Frostfell

Praetor Denik can be found in Lceanium and the Skyfire Mountains (group mission).

These are made with one using a Lcea Katta ornament and (1) to (5) Combine Viridian Chain, Cloth, Leather or Plate -- which, are also crafted using Sebilisian Viridian Pigment (a random EoK / RoS drop) and Combine Chain Links, Cloth Yard, Leather Square or Plate Sheet (sold by a records clerk in Lceanium) (Baking, Brewing, Jewel Craft, Pottery, Smithing and / or Tailoring Skill 350 to 450).

The book "Simple Secrets of the Combine Hero" (a random drop from named / rare MOB's in Empires of Kunark) can be scribed to learn all of these recipes.

Restless Ice

Violet is both a color (purplish-blue) and a flower.

The recipe book, "The Lost Art of Working with Wither and Decay" (which can be scribed to learn the recipes) is sold by the Seller of Salvaged Goods (Sul Vius: Demiplane of Decay), along with the required templates. This vendor is only up at night and can also appear in instances of the same zone.

The recipe is simply the template (see above) and a Chain, Cloth, Leather or Plate Scraps of Wither and Decay (rare drops in Sul Vius Demiplane of Decay).

Wolfkin's Northman
